&lt;P&gt;I suspect you might be hitting:&lt;/P&gt;
&lt;P&gt;PAN-OS 10.2.3-h9 Addressed Issues&lt;/P&gt;
&lt;P&gt;PAN-195149 Fixed an issue where firewall administrators were unable to log in to the web interface when RADIUS two-factor authentication was used.&lt;/P&gt;
&lt;P&gt;&amp;nbsp;&lt;/P&gt;
&lt;P&gt;We also hit it and this was addressed in some newer releases. I recommend taking a look at release notes or opening a support case.&lt;/P&gt;

      <title>Authentication Time out - RADIUS</title>

      <description>&lt;P&gt;All of our PA 820 are unable to login with RADIUS, both SSH and the web UI will time out. I see the access accept from my NPS server when I debug the authd log, but it then says&lt;/P&gt;
&lt;P&gt;===============AUTHD: start=========================&lt;/P&gt;
&lt;P&gt;ERROR: No such table: device DB.&lt;/P&gt;
&lt;P&gt;&amp;nbsp;&lt;/P&gt;
&lt;P&gt;Because this is impacting my entire network, I fell this is something on the NPS. This happened overnight, and nothing was changed when this issue started.&lt;/P&gt;
&lt;P&gt;&amp;nbsp;&lt;/P&gt;
&lt;P&gt;Any suggestions on how to better determine why the authentication is timing out?&lt;/P&gt;</description>
