Please check whether you have enabled "response pages option" checked for the interface management  profile that you have applied for the interface that is acting as ssl-vpn portal.

The management profile has the "response pages option" checked and it is assigned to the interface that is acting as ssl-vpn portal (loopback.1)

These are strange errors. Has this worked in the past or is it a new configuration?

If it has worked in the past, I would start checking health of the device - for example maybe disk space is low or out of memory

How is the health of the device?

A new feature was introduced in PAN OS 6.1 To disable the Login page on Portal

If you will Enable this Feature and when the user will visit to the portal by https://portal address the page will not be presented to him however with the GP client he will be able to connect

The error which the user will get with this option checked is
https://portal address
404 page not found

So if you want your users to see the Portal page kindly Disble the same
