I have checked some URL in urlfiltering.paloaltonetworks.com and category does not relate. I want to know How Palo consider the URL and put it to that category.

It's automated categorization primarily that take into account the content, features, and prevalence of the URL showing up in identified threat activity. Past that, anyone can submit a category change request and get that URL analyzed to confirm the selected categories.
Those change requests can be manually reviewed or automatic; it seems like there's some sort of low threshold of "hits" of a URL that allows it to be automatically reviewed and updated. Most of the time if you put in a category change request that's being reviewed by an actual human as I understand it.
