When using the Cryptocard authentication server we have had issues where no radius packets are coming through the PA500 on version 4.0.1. When the packet capture (tcpdump) was run no packets appeared on the interface.....
The only way to solve this problem was to upgrade to 4.0.4, the latest release. Magically this worked. I require some sort of explanation as to why this information did not go through, is this a known issue? Why would upgrading simply fix the problem? Would this occur on 4, 3 and 2 revisions of PA. fairly concerning.
There could be a variety of root causes for the issue that you describe, but without the device logs or a debugging session while you are experiencing the issue there is no way to be sure what was actually happening in this case.
